The Patient Navigator oversees a caseload of patients and acts as the primary liaison between the patient and scheduling their colonoscopy. The Patient Navigator is responsible for implementing the comprehensive care plan to each patient as determined upon enrollment in the program. The Patient Navigator ensures that the patient adheres to the established plan of care.
· Schedule direct access colonoscopies that are referred within the MS system – starting with uptown practices
o Review all triaged and approved direct access cases
o Contact patients to schedule colonoscopy
o Work to establish a relationship with the patient
o Remind patient about the need for COVID testing prior to the procedure
o Confirm that the patient is willing to do both the COVID test and the colonoscopy.
o Answer any questions/issues
o Provide comfort to patient anxieties
o Provide patient with instructions on prep for procedure, need for transportation and escort
o Schedule the procedure
o Provide ongoing navigation – reminder phone calls; addition review regarding COVID testing, the prep, escort, etc.
§ This will entail numerous phone calls
o Track all encounters in the tracking database
o Staff messages as needed
o Flexible work schedule – will need to work at least 2 evenings and some Saturdays
o Experience with Excel and other computer software
o BA degree or 3 years relevant experience
o Excellent communication and telephone skills
o Bilingual in Spanish
